The UH Community Organization was formed in 2011 with an aim of providing assistance to children living in the Sixth Ward to help them overcome challenges they face in order to excel in their academic performance. This grant proposal is therefore aimed at seeking funds from the Community-Campus Partnerships for Health in order to help these children accomplish their academic goals and excel in life (Carter & Quick, 2003; Ward, 2011).

Background of the Study/Problem

In the Sixth Estate in Houston, it was noted that a high number of children could not achieve their academic goals due to the surrounding environment. It was also noted that most of them in this area performed dismally at schools due to the lack of basic needs and most specifically the lack of food with a balanced diet. In view of this, the UH Community Organization was established in order to address these problems and assist children in realizing their potential. It was, therefore, necessary for the UH Community Organization to come up with a solution, which could ensure that children leaving in the Sixth Estate of Houston were well catered for (Reifer-Lehrer, 2005).

The UH Community Organization can greatly help the community that lives in and around the Sixth Estate, due to it will be able to educate people on the best ways of living healthy lives and to help them through the provision of some basic needs. The community will greatly benefit from the partnership with the UH Community Organization since it will become aware of various challenges affecting it and solutions towards eradicating such problems.

The CCHP Award will allow the UH Community Organization to foster a strong partnership between the community and the organization through the provision of funds, which will ensure that the operations of the latter go on smoothly. On the other and, the UH will be responsible for ensuring that the social, economic and general needs of people living in the Sixth Estate are well catered for.

The UH Community Organization strives to achieve policy changes and systems, which are required to overcome the major causes of social, health and economic inequalities. With such objectives in mind, the organization seeks to request for funding from the Community-Campus Partnerships for Health (Clarke & Fox, 2006).
